LICENSE BY ENDORSEMENT
Valley reimburses for state licenses with the following parameters:
- Valley is the first company the clinician is traveling or working for in that state (licensure only)
- The license was issued < 60 days of the clinician’s assignment start date
- Receipts are submitted to Valley within 7 days of the clinician’s start date
- The clinician completes a 13-week assignment with no fewer than 468 hours of work

*Temporary permits are issued when waiting to receive your permanent license. Other additional steps to receive permanent licenses must be in the process already.

ADDITIONAL INFORMATION
- Your West Virginia State Board of Nursing will expire on October 31st of each year
- If you obtained your license before November 1, 2012, you will be required to complete 12 hours of CE including 3 hours related to Substance Abuse unless exempt
- If you are licensed on or after Nov 1 2012, you will be required to complete 3 hours related to Substance Abuse unless you are exempt
