LICENSE BY ENDORSEMENT
Valley reimburses for state licenses with the following parameters:
- Valley is the first company the clinician is traveling or working for in that state (licensure only)
- The license was issued < 60 days of the clinician’s assignment start date
- Receipts are submitted to Valley within 7 days of the clinician’s start date
- The clinician completes a 13-week assignment with no fewer than 468 hours of work

*To speed up the licensure process, you may obtain electronic fingerprinting at the DOPL’s office. All documents required for your application must be received within 30 days.

ADDITIONAL INFORMATION
To obtain license you must have one of the following completed:
- You must complete 400 hours of licensed practice
- You must complete 200 hours of licensed practice and 15 hours of approved CEUs
- You must complete 30 hours of approved CEUs
You must submit a new application if your license has been expired for more than 2 years.
