LICENSE BY ENDORSEMENT
Valley reimburses for state licenses with the following parameters:
- Valley is the first company the clinician is traveling or working for in that state (licensure only)
- The license was issued < 60 days of the clinician’s assignment start date
- Receipts are submitted to Valley within 7 days of the clinician’s start date
- The clinician completes a 13-week assignment with no fewer than 468 hours of work

- Fill out online application
- Proof of your original state license
- Proof of your most recent or current state license
- Your nursing school Transcript (must come directly from your school)
- Provide proof of at least 960 practice hours in the last 5 years
- Completed fingerprint card and criminal background check.

ADDITIONAL INFORMATION
You must do the following to renew your nursing license:
- You will be required to complete 7 hours of pain management- related CEU within the first 24 months of renewal of your Oregon nursing license.
- You will be required to have a minimum of 960 hours of practice in the past 5 years, complete re-entry program within 2 years preceding renewal of your nursing license, OR graduated from OSBN-approved nursing program within 60 days of your license expiring
- If your license is expired by more than 60 days, you will be required to provide proof of a minimum of 960 hours of practice in the past 5 years and complete a background check
