COMPACT STATE
LICENSE BY ENDORSEMENT
Valley reimburses for state licenses with the following parameters:
- Valley is the first company the clinician is traveling or working for in that state (licensure only)
- The license was issued < 60 days of the clinician’s assignment start date
- Receipts are submitted to Valley within 7 days of the clinician’s start date
- The clinician completes a 13-week assignment with no fewer than 468 hours of work

*Temporary permits are issued when waiting to receive your permanent license. Other additional steps to receive permanent licenses must be in the process already.
After completing the endorsement application, you will need to take the KY Jurisprudence Exam online.

ADDITIONAL INFORMATION
You will be required to pass the Federal and State background check for reinstatement. If your license has lapsed for one year or more, you will be required to take the KY jurisprudence exam. In addition, you will also need to provide verification of employment for employment outside of Kentucky for at least 500 hours of work within the last 5 years.
