LICENSE BY ENDORSEMENT
Valley reimburses for state licenses with the following parameters:
- Valley is the first company the clinician is traveling or working for in that state (licensure only)
- The license was issued < 60 days of the clinician’s assignment start date
- Receipts are submitted to Valley within 7 days of the clinician’s start date
- The clinician completes a 13-week assignment with no fewer than 468 hours of work

ADDITIONAL INFORMATION
You must also complete the following to reinstate your nurse license:
- You must notarize your application
- If your nurse license has lapsed within 5 years, you must provide proof of 30 hours of approved continuing nurse education within 2 years
- If your nurse license has lapsed more than 5 years, you must provide verification of current active license and proof of 30 hours of approved continuing nurse education within 2 years
*You are able to receive a 120 day temporary nurse permit in order to complete the 30 hours of continuing education.
