LICENSE BY ENDORSEMENT
Valley reimburses for state licenses with the following parameters:
- Valley is the first company the clinician is traveling or working for in that state (licensure only)
- The license was issued < 60 days of the clinician’s assignment start date
- Receipts are submitted to Valley within 7 days of the clinician’s start date
- The clinician completes a 13-week assignment with no fewer than 468 hours of work

ADDITIONAL INFORMATION
If your license has been expired for more than 18 months, you will need to contact the Indiana State Board of Nursing for renewal information.
If your license has been expired for more than 3 years, you will have to complete an expired renewal form and include a copy of current license from another state
